Survey Learning Objects are used to collect Learner sentiments and opinions. This article includes instructions on how to configure a Survey Learning Object.

Instructions: Add Survey Object
To add a Survey Object to an Online Course, follow these steps:
- Access the Admin Experience as an Administrator.
- Open the Courses menu from the left-hand side and click Courses.
- From the Courses Report either add a new Online Course or Edit an existing one.
- From the Add/Edit Online Course page navigate to the Syllabus section and click Add Learning Object.
- From the Add Learning Object modal frame, select Survey and then click Next.
- Configure your Survey to fulfill the needs of your use-case.
- Save when you are complete.

Form Section: Details
Field | Details |
Name |
Enter a Name to identify the Learning Object. The information entered is visible to Learners within the Learner Experience and Administrators in the Admin Experience. This is a required field. |
Description | Enter a description to provide additional details about the Learning Object. The information entered is visible to Learners within the Learner Experience and Administrators in the Admin Experience. |
Form Section: Options
Click the toggle buttons to adjust the state. The default state for the following options is OFF.

Allow Multiple Attempts | Allow the User to attempt the Learning Object multiple times. |
Randomize Question Order | Questions are asked in random order. |
Randomize Answer Order | Answer options are displayed in random order. |
Use Answer Based Actions | Answers can trigger immediate actions upon selection. |
Allow Scored Answers* |
Allows you to provide feedback and actions based on ranges of total response points. *This feature has not been updated for the Admin Refresh. |
Single Page Layout | Turning this option on will change the layout to a single page. |
Show Navigation | Show the Navigation Bar. |
Form Section: Messages
You can configure two different Message states for Surveys: Introduction and Post Message.

Introduction Message | The text entered into the field is presented to Learners before they begin the Survey. |
Post Message | The text entered into the field is presented to Learners after they complete the Survey. |
Form Section: Questions
The Questions section is where you configure Survey Questions. Use the Add Question Group option to group Questions together and organize the flow of the Survey.

The default value is Question Group 1. You can change it by typing a new value into the Group Name field. Underneath this field, are two selections that determine how the Question Group will function:

Manage Questions | Click this button to open a new form to add and edit the Survey Questions. Click here for continued instruction. |
Add Question Group | Click this button to create an additional Question Group for the Survey. |
Instructions: Manage Questions
- Click the Manage Questions button. This Action updates the modal to the Manage Questions form.
- Click the Add Question button. This Action updates the modal to the Add Questions form.
- When you are done configuring the Add Survey form, click the Apply button.
- This Action will save the entered information and create the new Survey Learning Object.
- The modal will close, and you will return to the Add Online Course page.
